FHIR DS4P is intended as a platform upon which regional IGs would be written that need the extensions and profiles defined in DS4P. As such, there is little to be gained by pointing at FHIR DS4P.<br /><br />The security pages in R4B include many improvements over R4. Including working references to the security tagging vocabulary.<br /><br />However, these both are way too broad to be useful as a step from no Privacy controls to having useful Privacy controls. This is where the IHE Privacy Consent on FHIR (PCF) Implementation Guide helps greatly. In fact it provides very useful Privacy controls that do NOT rely on security tagging. Security tagging is the ultimate, but is a big step and often very hard to implement. Security tagging requires some architecture implementation of a Security Labeling Service, and that likely needs assistance from CDS. (See Appendix P in the PCF). The PCF brings Consent, and data identification together into a functionality delivering Privacy controls. <br /><br />Yes, PCF is not yet in Trial Implementation, but will be by the time the commenting period resolves. <br /><br />https://profiles.ihe.net/ITI/PCF/1.0.0-comment/index.html<br /><br />Break-Glass: I am not convinced that this is a topic that needs to be addressed by ONC. The specific use-cases are clear, but they are all within an organization. The need for application of break-glass at the API or HIE level is unclear. This said, the PCF has a start at addressing this, but does not have a clear winner for "the breaking of the glass", which is very application specific today.<br /><br />Really wish I could find a place to improve Privacy control of Transparency. the IHE BALP provides for auditEvent profiles of when data are used, that could be made available to a Patient so that they can understand when and why and by who, their data are used. I've been getting them for a little over $2USD each online and they are by far the most comfortable N95's I've found. A few things I wanted to point out:<br />1) There’s a step to verify that the person is qualified to get the vaccine based on age/occupation/risk factors<br />2) You missed the documentation step. We have to fill out a standard paper card for each that we administer. While in theory that could be printed, it’s supposed to be a standard card size and card stock. So a label printer is a better solution. But it’s a non-standard tiny label so we weren’t able to do that without purchasing lots of new label printers. Adding two *standard* methods to do the same thing reduces interoperability, as you increase the odds that two parties will support different methods, and not be able to talk to each other.
